CHECKLIST FOR PROJECTS INVOLVING HUMAN PARTICIPANTS IN RESEARCH
It is important that BUREC receives sufficient information to review your application. Please provide the following:
- Name of principal investigator (PI) including address and phone number. If PI is graduate student, also include name and contact number of supervisor
- Title (10 words or less) of project
- Brief description of the project's objectives (Use language that can be understood across disciplines)
- A copy of the statement about the project which includes the following:
- A full description of all procedures, instruments, and expectations including information about the time requirements for participation
- Status and names of persons who are involved in collecting data for project (if known)
- Any risks or benefits likely to arise from participation in the project
- Assurance that the participant(s) understand that participation is entirely voluntary, and that they can withdraw at any time without giving a reason
- Assurance that non-participation will have no impact on grades, class standing, employment or result in any other penalty
- Means by which data will be kept confidential
- Means by which the anonymity of participants will be protected
- Details of any compensation or benefits offered to subjects (if applicable)
- Details regarding storage and retention (data must be kept by the PI for a minimum of five years)
- Method of obtaining permission to audio or video tape subjects (if applicable)
- How tapes, notes or other raw data will be disposed of at the end of the project
- How the data will be used for presentation or dissemination and/or by third parties (if applicable)
Please attach a copy of the following:
- Questionnaire, test items, sample tasks, or interview questions
- Letters of initial contact with subjects/participants; parents/guardians; institutions, agencies, companies; school boards, principals, teachers, etc.
- Consent forms for subjects/participants; parents/guardians; institutions, agencies, companies, etc.; school boards, principals, teachers, etc.
- Consent to audio or video tape subjects, interviews, etc.
- Confidentiality agreement for persons involved with collecting and/or handling data
- Ethics Certificate - your signature indicates you agree to abide by all policies, procedures, regulations and laws governing the ethical conduct of research involving humans.
Please forward an original and six (6) copies of your application and attachments to the Brandon University Research Ethics Committee c/o the Office of the Vice-President Academic & Research.
NOTE: Only two copies of the Ethics Certificate should be attached.
